Due to the ongoing coronavirus outbreak Chertsey Health Centre remains open as normal from 8am to 6.30pm. however please do not come to the surgery unless you have a pre-booked appointment.
If you need to contact the practice please complete our contact the practice form. If you are requesting to see the Doctor you will be offered a triage appointment and if the doctor feels a face to face appointment is required they will arrange that with you during your triage call.
We are able to send sick certificates and paperwork electronically to your phone or email.
All prescriptions are sent electronically to your nominated pharmacy. If you would like to set up a nominated pharmacy please complete our electronic prescribing form.
We are trying to offer as many normal services as we can and do have nurse appointments available, although there may be a wait as we deal with the demand to clear the back log. To request an appointment with the nurse please complete our request form.
